Implementation of Transition workers to support young peoples transition from care to independence.

Action for Children have committed to undertaking initiatives to support young people and their transition from care to Independence. 

Action for Children will hire transition workers across five UK locations, for an initial period of two years, with funding from our corporate partner CBRE. Each transition worker will provide intensive support to ten young people per year, aged from 15 to 25.

The transition workers will provide support before, during, and after the young person’s transition from care into independent living to help them to secure safe housing, build aspirations and gain employment, education or training opportunities, and to support their emotional wellbeing.
